Position Overview

The RN Clinical Coordinator plays a central role in managing members with complex medical and behavioral needs. This position focuses on care coordination across clinical, behavioral, and socioeconomic domains to ensure appropriate service utilization and improved care quality.

This is a field-based role, requiring approximately 80 percent travel across Mercer County, New Jersey, including member homes and long-term care facilities. The ideal candidate is adaptable, patient, and experienced in engaging diverse populations.


Key Responsibilities

  • Conduct comprehensive needs assessments through face-to-face or telephonic engagement, covering medical, behavioral, functional, cultural, and socioeconomic factors.

  • Develop and implement person-centered care plans aligned with evidence-based standards, focusing on chronic disease management, wellness promotion, medication safety, and social determinants of health.

  • Collaborate with internal teams, healthcare providers, and community partners to execute care plans and coordinate support services.

  • Provide patient education, health coaching, and guidance to promote self-management and lifestyle improvements.

  • Lead proactive discharge planning and manage care transitions following hospital, emergency room, or skilled nursing facility encounters.

  • Advocate for member rights, representing their needs and preferences within healthcare settings.


Required Qualifications (Experience Mandatory)

  • Active, unrestricted Registered Nurse (RN) licensure.

  • Minimum 2 years of clinical experience.

  • Minimum 1 year of experience using Microsoft Word, Excel, and Outlook.

  • Reliable transportation with the ability to travel up to 80 percent within Mercer County.


Preferred Qualifications

  • Bachelor of Science in Nursing (BSN) or Master’s degree in a clinical field.

  • CCM certification (Case Management).

  • Minimum 1 year of community-based case management supporting individuals with complex needs.

  • Experience in interdisciplinary or team-based care.

  • Background in managed care or utilization management.


Physical Requirements

  • Ability to transition between office and field sites multiple times daily.

  • Ability to navigate various home and community environments during visits.

  • Ability to carry required equipment, including laptop and clinical tools.

  • Ability to sit for extended computer-based work sessions.


Compensation and Benefits

  • Hourly pay range: $28.27 to $50.48 (based on experience, education, certifications, and local market conditions).
