Schedule:

  • Monday-Friday: 7 AM - 3 PM, 10:30 AM - 6:30 PM, 11:30 AM - 7:30 PM (Rotating)
  • Rotating Saturdays: 8 AM - 4 PM
  • Rotating Sundays: 8 AM - 2 PM

Responsibilities:

  • Evaluate physician medication orders for appropriateness, including dosage, interactions, and routes of administration, and provide therapy recommendations to physicians and facility staff.
  • Maintain an up-to-date patient/resident database, including clinical and laboratory data and concurrent medications.
  • Ensure necessary laboratory tests for high-risk conditions are ordered, monitored, and discussed with the healthcare team.
  • Collaborate with the healthcare team to provide clinical and technical support as needed.
  • Document all communication with healthcare team members, patients, residents, and families in the medical record.
  • Implement policies for the safe procurement, storage, distribution, and disposal of drugs.
  • Maintain all required documentation in accordance with federal and state regulatory agencies.
  • Report medication-related errors to the Pharmacy Manager.
  • Assist staff in safe and effective methods of drug procurement, maintenance, administration, and disposal.
  • Perform other assigned tasks.

Qualifications:

  • Bachelor's Degree in Pharmacy; Pharm D preferred
  • Minimum one year of experience in long-term care
  • Licenses and certifications as required by government regulations and business needs
  • Strong attention to detail, goal-oriented, team player, with excellent oral communication skills
