Role Overview:

The Medication History Pharmacy Technician assists in obtaining accurate medication histories for patients being admitted to the hospital. Working under the supervision of a pharmacist, the technician ensures that prescription and allergy information is gathered through patient/family interviews, and by contacting pharmacies, long-term care facilities, and prescribers when necessary. The technician ensures adherence to hospital and pharmacy department policies, safety practices, and regulatory requirements.

Key Responsibilities:

  • Collect accurate medication histories, allergy information, and related documentation.
  • Conduct patient or family interviews to gather medication information.
  • Contact pharmacies, long-term care facilities, and prescribers as needed.
  • Follow pharmacy department policies and procedures for medication history documentation.
  • Maintain competencies and ensure safety practices are met in accordance with pharmacy guidelines.

Qualifications:

  • Education: High School Diploma or GED required.
  • Certification/Licensure: Certified Pharmacy Technician (CPhT) with the State of Ohio Board of Pharmacy.

Schedule: Full-time with evening shifts.
Hours: 40 hours per week.
