Pharmacist Position
Location: Samuel Simmonds Memorial Hospital

Brief Summary:
Provide accurate, safe, efficient, and cost-effective pharmaceutical care directly or indirectly through supervised support staff to patients at Samuel Simmonds Memorial Hospital and retail consumers.

Essential Job Functions:

  • Act as a specialist in a designated area of expertise, engage in consumer education, and provide clinical practice direction to staff.
  • Aid in developing, implementing, and evaluating treatment guidelines for cost-efficient drug therapy.
  • Participate in interdisciplinary groups to enhance patient care.
  • Assist in Performance Improvement, Drug Utilization Evaluation, Adverse Drug Reaction reporting, Medication Use, and other projects as assigned by the Director of Pharmacy.
  • Provide and evaluate staff development opportunities for Pharmacy Technicians and assess their clinical skills.
  • Conduct and participate in clinical research.
  • Deliver accurate pharmaceutical distribution services by interpreting drug orders, entering them into the pharmacy system, and dispensing medications.
  • Identify, resolve, and prevent drug-related problems by reviewing patient data for interactions, allergies, and adverse reactions.
  • Communicate with physicians, nurses, and healthcare providers regarding patient drug therapy.
  • Document drug information provided to patients and representatives.
  • Monitor drug therapy for cost-effectiveness and recommend savings to physicians.
  • Maintain an updated drug knowledge database and pursue advanced practice through publications, presentations, and professional development.
  • Document medication errors and near misses in Quatro's.
  • Stay informed on state and federal pharmacy regulations and practice accordingly.
  • Supervise support personnel in compliance with state laws to ensure accurate preparation and dispensing of pharmaceuticals.

Education & Experience:

  • Bachelor's degree in Pharmacy required; Pharm.D. preferred.
  • Completion of a residency and a minimum of two years post-graduate clinical practice experience, or equivalent retail/inpatient pharmacy experience preferred.
  • Advanced Cardiac Life Support certification and active membership in state and national pharmacy organizations preferred.

Certification/Licensure:

  • Valid unrestricted license to practice pharmacy and dispense controlled substances in the United States.
