Job Responsibilities

Clinical Services:

  • Provide therapeutic recommendations to nurses, physicians, and other hospice personnel.

  • Conduct and evaluate medication profiles/histories, assess compliance, and suggest modifications to optimize regimens.

  • Apply sound clinical pharmacy principles, including pharmacodynamic and pharmacokinetic considerations.

Customer Interaction:

  • Respond to patient and caregiver questions promptly and professionally.

  • Address and resolve customer issues with a focus on satisfaction and care.

Operational Duties:

  • Transcribe, verify, and fill medication orders.

  • Maintain proper storage and security conditions for medications.

  • Keep pharmacy workspaces neat and error-free.

Team Collaboration:

  • Work cooperatively with pharmacy technicians and other pharmacists.

  • Foster professionalism and courteousness in all interactions.


Qualifications

Education and Licensure:

  • Doctor of Pharmacy (PharmD) degree or BS in Pharmacy.

  • Current and active pharmacist license in good standing with the State Board.

Skills and Competencies:

  • Strong written and verbal communication, particularly effective telephone correspondence.

  • Excellent problem-solving abilities and the ability to prioritize effectively.

  • Capable of working independently while maintaining professionalism under stress.

  • Team-oriented with a customer-first attitude.
