Job Responsibilities
Clinical Services:
Provide therapeutic recommendations to nurses, physicians, and other hospice personnel.
Conduct and evaluate medication profiles/histories, assess compliance, and suggest modifications to optimize regimens.
Apply sound clinical pharmacy principles, including pharmacodynamic and pharmacokinetic considerations.
Customer Interaction:
Respond to patient and caregiver questions promptly and professionally.
Address and resolve customer issues with a focus on satisfaction and care.
Operational Duties:
Transcribe, verify, and fill medication orders.
Maintain proper storage and security conditions for medications.
Keep pharmacy workspaces neat and error-free.
Team Collaboration:
Work cooperatively with pharmacy technicians and other pharmacists.
Foster professionalism and courteousness in all interactions.
Qualifications
Education and Licensure:
Doctor of Pharmacy (PharmD) degree or BS in Pharmacy.
Current and active pharmacist license in good standing with the State Board.
Skills and Competencies:
Strong written and verbal communication, particularly effective telephone correspondence.
Excellent problem-solving abilities and the ability to prioritize effectively.
Capable of working independently while maintaining professionalism under stress.
Team-oriented with a customer-first attitude.
