Diabetes Care & Education Specialist

Location: Montgomery, Cincinnati, OH
Position Type: Full-time, Day Shift


Job Overview

We are looking for a Certified Diabetes Care and Education Specialist (CDCES) to deliver comprehensive and patient-centered care, bridging the gap between clinical and self-management aspects of diabetes and cardiometabolic care. As a vital member of the interprofessional team, the CDCES will collaborate to educate and support patients, advocating for better health outcomes and improved quality of life.


Key Responsibilities

  • Provide quality diabetes self-management education and medical nutrition therapy (for Registered Dietitians) in individual and group settings (both in-person and virtual).
  • Conduct comprehensive assessments, develop individualized care plans, and promote successful self-management based on patients' needs and goals.
  • Interpret personal health data and educate patients about relevant labs, including diabetes and comorbidities like cardiometabolic and renal health.
  • Actively contribute to multidisciplinary team discussions to identify resources, strategies, and interventions for better diabetes self-management.
  • Utilize technology to assist in patient education and manage a patient panel, tracking progress and updating providers.

Job Requirements

  • Education: Bachelor’s Degree in Nursing or Dietetics/Nutrition.
  • Licensure: Registered/Licensed Dietitian or Registered Nurse.
  • Experience: 2-3 years of experience in diabetes care and education.
  • Certification: Must hold CDCES certification or be eligible to sit for the examination within 24 months of hire.

Working Conditions

  • Physical Demands: Minimal lifting (rarely over 10 lbs); frequent sitting, standing, and walking.
  • Skills & Abilities: Requires consistent use of critical thinking, interpersonal communication, and technology for patient care management.
