Job Overview:
The Clinical Pharmacy Specialist is responsible for ensuring the safe and effective use of medications within a collaborative, multidisciplinary healthcare environment. The specialist will work closely with the pharmacy team, prescribers, and other healthcare providers to deliver high-quality, safe medication therapy to pediatric patients. This role involves assessing patient needs, adjusting treatments based on patient responses, providing education, and preventing drug-related issues. The Clinical Pharmacy Specialist will also contribute to medication management leadership, education, and research within the department.

Key Responsibilities:

  • Promote safe medication use by collaborating with the pharmacy team and healthcare providers.
  • Provide individualized pharmaceutical care, considering age, disease-specific characteristics, and patient response.
  • Identify, resolve, and prevent drug-related problems.
  • Serve as a departmental resource, acting as a liaison to other teams and committees.
  • Lead the development of evidence-based medication strategies and drug-use policies.
  • Provide patient education and assist in clinical interventions to manage adverse medication events.
  • Mentor pharmacy residents and students through preceptorship roles.
  • Participate in quality care initiatives and support research within the pediatric setting.

Minimum Qualifications:

  • Education: Doctor of Pharmacy (Pharm.D.) or Bachelor's degree in Pharmacy required.
  • Licensure: Eligible for licensure as a pharmacist in the state of Ohio, to be obtained prior to start date.
  • Experience:
    • Completion of an ASHP-accredited PGY2 Pharmacy Residency in Pediatrics or a relevant specialty.
    • OR Completion of an ASHP-accredited PGY1 Pharmacy Residency with a pediatric focus.
    • OR 4 years of hospital experience with pediatric patients.

Certifications/License:

  • Registered Pharmacist (RPh) - Ohio State Board of Pharmacy
