About the Role

Clinical Pharmacist Specialists are integral to a patient care team, focusing on safe, effective medication management for specialized patient populations. They provide direct patient-centered care, collaborate with multidisciplinary teams, and ensure that clinical pharmacy services are seamlessly integrated into care delivery.

Key Responsibilities:

  • Lead patient service lines, therapeutic programs, and clinical areas.
  • Facilitate pharmaceutical care services, direct patient care programs, and medication utilization systems.
  • Design and implement stewardship activities and restriction/surveillance programs.
  • Track and evaluate pharmacy programs for operational, quality, and financial efficiency.
  • Identify practice-related issues, initiate clinical research projects, and support quality improvement initiatives.
  • Develop policies for drug purchasing, usage, distribution, and control.
  • Provide decentralized patient care, actively collaborating with care teams.
  • Identify high-risk patients and make evidence-based, patient-centered medication recommendations.
  • Participate in medical emergencies and provide discharge medication review and counseling.
  • Maintain proficiency in hospital computer systems, medication ordering, and operational programs.
  • Oversee pharmacy operations, including specialty medication procurement and dispensing (e.g., chemotherapy, parenteral nutrition).
  • Facilitate experiential education, mentorship, and staff training for students and residents.

Qualifications:

  • Education:
    • Graduate of an ACPE-accredited College of Pharmacy.
    • PharmD preferred.
  • Licensure:
    • Current pharmacist licensure in Pennsylvania (or eligibility).
  • Experience:
    • Completion of ASHP-accredited PGY-2 Residency OR
    • PGY-1 Residency + 3 years clinical pharmacy experience in a specialized area OR
    • 6 years clinical pharmacy experience in a specialized area.
  • Certification:
    • BPS Board Certification preferred (recognized specialties include Ambulatory Care, Oncology, Critical Care, etc.).

Licensure, Certifications, and Clearances:

  • Pharmacist licensure in Pennsylvania.
  • Act 34 with renewal compliance.
