Job Title: Clinical Pharmacist Specialist


About the Role

As a Clinical Pharmacist Specialist, you will be pivotal in ensuring optimal medication therapy for specialized patient populations and improving patient outcomes through close collaboration with physicians, nurses, and other healthcare team members.

This role primarily operates during the day shift but may require some evening and night hours as needed, along with rotating weekend and holiday shifts.


Key Responsibilities

  • Lead and Innovate: Oversee patient service lines, clinical areas, and therapeutic programs.
  • Patient Care: Provide direct patient care, including medication review, reconciliation, and counseling.
  • Collaboration: Actively participate as a member of a multidisciplinary team to provide patient-centered care.
  • Quality Improvement: Identify practice-related issues and lead clinical research projects and quality improvement initiatives.
  • Education and Mentorship: Precept pharmacy students and residents, and develop training experiences.
  • Operational Excellence: Maintain proficiency in decentralized pharmacy services and clinical programs while overseeing pharmacy operations.

Qualifications

  • Graduate of an ACPE-accredited College of Pharmacy.
  • B.S. in Pharmacy or Doctor of Pharmacy (PharmD) required; PharmD preferred.
  • Current licensure as a Pharmacist in practicing state.
  • Completion of an ASHP-accredited PGY-2 Pharmacy Residency OR PGY-1 Pharmacy Residency plus 3 years of clinical pharmacy experience in a specialized area OR 6 years of clinical pharmacy experience in a specialized area.
  • Specialized knowledge in advanced pharmacy practice, comprehensive medication management, and evidence-based medicine in your clinical specialty.
  • BPS Board Certification preferred (recognized specialties include: ambulatory care, cardiology, compounded sterile preparations, critical care, emergency medicine, geriatric, infectious diseases, nuclear, nutrition support, oncology, pediatric, pharmacotherapy, psychiatric, and solid organ transplantation).
