Clinical Pharmacist Position at Lake Health District

Position Details:

  • Hours: Monday to Friday, 7:00 AM - 4:00 PM during training. Must be flexible as needed.
  • Benefits: Coverage eligibility begins the 1st of the month following the date of hire.

Position Summary:
The Clinical Pharmacist is responsible for providing safe, effective, and prompt medication therapy to District patients. This role includes supporting medication-use systems and ensuring optimal therapy for patients across a broad range of disease states. The pharmacist will collaborate with physicians, nurses, and hospital personnel to deliver direct patient care, integrating pharmaceutical care programs throughout the hospital. Responsibilities include assessing patient needs, adjusting care based on responses, and preventing medication-related adverse events. Additionally, the Clinical Pharmacist will serve as a resource for other departments and participate in quality management programs aimed at improving pharmacy services.

Responsibilities:

  • Provide comprehensive and individualized pharmaceutical care.
  • Assess and adjust drug therapy according to patient responses.
  • Conduct clinical interventions to prevent medication adverse events.
  • Collaborate with healthcare teams to optimize patient care.
  • Participate in quality management and regulatory compliance initiatives.
  • Provide education and training related to medication therapy and pharmacy practice.

Requirements:

  • Education: Graduate of an ACPE-accredited College of Pharmacy.
  • Licensure/Certification: Licensed (or eligible for licensure) in the state of Oregon.
  • Experience: Clinical pharmacy experience in a hospital setting preferred.

Employment Requirements:

  • Must be a U.S. Citizen or National.
  • Satisfactory background investigation and fingerprint check.
  • Successful completion of a 500-hour probationary period.
  • Criminal background check and pre-employment drug screen required upon conditional job offer.

  • Submit DD214 or certification if claiming veteran’s preference.
