Clinical Pharmacist Position at Lake Health District

Position Details

  • Hours: Monday to Friday, 7:00 AM - 4:00 PM during training. Flexible schedule required.

Position Summary
The Clinical Pharmacist at Lake Health District is responsible for providing safe, effective, and timely medication therapy to patients with a variety of disease states. This role involves collaborating with physicians, nurses, and hospital personnel to deliver optimal medication therapy. The pharmacist will be integrated into interdisciplinary programs and specialty services to ensure comprehensive pharmaceutical care, including assessing patient needs, adjusting care according to patient responses, and preventing medication-related adverse events. The Clinical Pharmacist will also participate in quality management initiatives and assist in improving medication-use processes and pharmacy practice.

Responsibilities:

  • Provide direct patient care and medication therapy services.
  • Collaborate with healthcare teams to optimize patient treatment.
  • Participate in the quality management program, including monitoring and documenting quality improvement projects.
  • Offer education and training on medication therapy and pharmacy practice.

Requirements

  • Education: Graduate of an ACPE accredited College of Pharmacy.
  • Licensure: Licensed (or eligible for licensure) in the state of Oregon.
  • Experience: Clinical pharmacy experience in a hospital setting preferred.

Employment Requirements:

  • U.S. Citizenship or National required.
  • Satisfactory background investigation and fingerprint check.
  • Successful completion of a 500-hour probationary period.
  • Criminal background check and pre-employment drug screen required upon conditional job offer.

Veteran’s Preference:

  • If claiming veteran's preference, submit a DD214 or certificate of expected discharge from active-duty service under honorable conditions.
