Position: Clinical Pharmacist
Location: Lake Health District, Oregon
Schedule: M-F, 7:00 AM - 4:00 PM (Training Phase)
Benefits: Full benefits begin the 1st of the month following the hire date.


Position Summary

The Clinical Pharmacist will deliver safe, effective, and timely medication therapy to patients in the district. As part of an interdisciplinary team, the pharmacist will collaborate with physicians, nurses, and hospital staff to provide optimal pharmaceutical care. Responsibilities include assessing patient needs, tailoring drug therapy, preventing adverse drug events, and ensuring compliance with quality and regulatory standards. Additional duties involve quality improvement initiatives, staff education, and serving as a resource to other departments.

Key Responsibilities

  • Provide individualized pharmaceutical care for a range of disease states.
  • Assess and adjust medication therapy based on patient-specific needs and responses.
  • Deliver patient education regarding medications and therapy plans.
  • Perform clinical interventions to improve patient outcomes and prevent medication errors.
  • Conduct quality management and regulatory compliance initiatives.
  • Provide education/training on medication use processes and quality improvement projects.

Requirements

  • Education: Graduate of an ACPE-accredited College of Pharmacy.
  • Licensure: Must be licensed or eligible for licensure in Oregon.
  • Experience: Preferred experience in a hospital setting.

Employment Conditions

  • U.S. Citizenship or National status required.
  • Completion of a 500-hour probationary period.
  • Successful adjudication of a background investigation and fingerprint check.
  • Pre-employment drug screening required.
