Position Overview

The LPN Intake Coordinator plays a critical role in home health operations by managing referral intake processes, verifying clinical and insurance information, and coordinating communication between referral sources and internal clinical teams.

This role requires strong clinical knowledge, insurance verification expertise, and the ability to manage high-volume referral workflows with accuracy and professionalism.


Key Responsibilities

Referral Intake & Clinical Coordination

  • Receive and analyze incoming referral and physician order calls.

  • Collect and document demographic and clinical information from clients, physician offices, hospitals, and other referral sources.

  • Compile and process referrals in accordance with regulatory and organizational standards.

Compliance & Verification

  • Verify referring physician licenses for all incoming referrals.

  • Research and validate insurance coverage or payer sources.

  • Confirm commercial policy eligibility and HIQA requirements where applicable.

Patient & Provider Communication

  • Educate prospective patients, families, physicians, and hospitals regarding available home health services.

  • Serve as a liaison between clinical teams and marketing staff to ensure seamless coordination.

  • Communicate referral updates clearly and accurately to appropriate internal teams.


Required Qualifications

  • Current and unrestricted Licensed Practical Nurse (LPN) licensure in the state of practice (New Jersey).

  • Current CPR Certification.

  • Minimum 1+ year of experience in insurance verification, medical office operations, home health, or healthcare administration.

  • Valid Driver’s License, active vehicle insurance, and access to reliable transportation or public transit.


Preferred Skills & Competencies

  • Knowledge of home health regulations and payer guidelines.

  • Experience with insurance eligibility verification and referral management systems.

  • Strong documentation and organizational skills.

  • Effective communication skills with patients, families, physicians, and internal stakeholders.

  • Ability to manage multiple referrals simultaneously in a fast-paced environment.


Compensation & Benefits

  • Hourly Pay Range: $14.00 – $27.69 per hour (based on experience, education, certifications, and local labor market conditions).
