Position Overview:
Join Sutter Health and be part of a dedicated healthcare team! We are seeking a Licensed Pharmacist to work independently and collaboratively with physicians and other healthcare providers to ensure optimal medication therapy for patients. The role focuses on patient counseling, medication reconciliation, monitoring, and managing disease states in a team-oriented care environment. The Pharmacist will work with high-risk populations, providing comprehensive medication management, education, and interventions for chronic diseases.

Responsibilities:

  • Conduct patient interviews and reviews of medical records and medication profiles.
  • Provide education and counseling to patients and healthcare staff.
  • Monitor adverse drug reactions and resolve medication-related issues.
  • Assist in medication therapy management, focusing on disease state management and coordination of care.
  • Collaborate with physicians for medication-related decision-making.
  • Participate in medication use evaluation and continuous improvement processes.

Education & Experience:

  • Education: Doctorate degree in Pharmacy from an accredited institution.
  • Experience: At least 5 years of recent, relevant experience as a Pharmacist.

Licensing & Certification:

  • Licensure: Current and active Pharmacist license (PHARMD or PHARMR).

Position Details:

  • Shift: Full-time, Monday to Friday, 8-hour shifts (No weekend work required).
  • Location: PAMF-Palo Alto Medical Foundation.
  • Employee Status: Regular, Non-Exempt.
  • Compensation: $90.94 to $104.56 per hour (salary range varies by location, factors such as experience and qualifications).
