Inpatient Pharmacist Position in Martinez, CA
Contra Costa County Department of Health

We are seeking an experienced Inpatient Pharmacist for a full-time role at our 175-bed community hospital in Martinez, CA. This position offers variable day and afternoon shifts and requires a minimum of 1+ years of pharmacy experience.

Essential Duties:

  • Review, process, and dispense physician medication orders and prescriptions.
  • Prepare and supervise the preparation of IV admixtures and chemotherapy.
  • Provide therapeutic drug monitoring consultations, including determining appropriate loading and maintenance doses based on patient-specific parameters and disease processes.
  • Maintain routine workflow, integrate non-routine tasks, and relay information as necessary to the incoming shift.
  • Consistently monitor and follow up on unresolved medication order issues.
  • Monitor lab orders and departmental messages, following up as needed.
  • Document clinical interventions accurately in the EHR system using provided guidelines.
  • Utilize knowledge and experience with Pyxis and other electronic medication stations.
  • Monitor and supervise technician activities, checking all medications before they are filled into medication stations.
  • Ensure the availability and maintenance of all emergency drugs during your shift.

Qualifications, Skills, and Experience:

  • Bachelor’s degree in Pharmacy or PharmD.
  • At least 1 year of experience in an inpatient pharmacy.
  • Active California Pharmacist License.

Compensation:

  • Starting at $70.00/hour.
