Job Title: Pharmacist (Float Pool)

Location: St. Elizabeth Healthcare, Kentucky and Indiana
Employment Type: Full-Time, Regular
Scheduled Hours: 35 hours per week
Shift: 7 On/7 Off (third shift, supporting acute care facilities)
FLSA Status: Non-Exempt


Job Summary:

This position is part of the Pharmacist Float Pool, responsible for floating to all required St. Elizabeth Healthcare (SEH) facilities. The pharmacist will provide pharmaceutical care, focusing on clinical monitoring, therapeutic interventions, and patient outcomes. The position also includes the dispensing and monitoring of medication, supporting safety initiatives, and ensuring medication compliance.


Key Responsibilities:

  1. Medication Order Verification:

    • Review and interpret medication orders, ensuring they are complete and appropriate for the patient's condition.
    • Check for drug interactions, allergy alerts, and duplications of therapy.
    • Clarify incomplete orders and suggest therapeutic alternatives.
    • Verify pediatric and chemotherapy orders.
  2. Therapeutic Clinical Monitoring:

    • Participate in ongoing medication therapy monitoring.
    • Contact prescribers with therapeutic alternatives and document recommendations in patient charts.
    • Ensure compliance with Collaborative Care Agreements and medication management protocols.
  3. Compounds and Dispenses Pharmaceuticals:

    • Ensure timely dispensing of correct medications.
    • Supervise the compounding and dispensing process, following USP standards and verifying ingredient compatibility.
    • Ensure the correct labeling, expiration dating, and barcode technology use for accurate dispensing.
  4. Supervision and Mentorship:

    • Actively supervise and mentor pharmacy interns and technicians.
    • Ensure timely completion of tasks and redirect staff when necessary to meet patient needs.
  5. Medication Use Safety Initiatives:

    • Detect and report medication variances and adverse drug reactions.
    • Participate in safety initiatives to improve medication use.
  6. Controlled Substance Accountability:

    • Supervise the receipt, dispensing, and distribution of controlled substances.
    • Ensure controlled substance inventories are accurate and reconcile discrepancies.
  7. Financial Contribution:

    • Encourage formulary compliance and suggest therapeutic alternatives for non-formulary requests.
    • Actively participate in IV to PO conversions to optimize medication use.
  8. Drug Information and Staff Development:

    • Provide accurate drug information to medical staff and patients.
    • Serve as a preceptor and develop staff education programs.
  9. Maintaining Professional Competency:

    • Maintain licensure and complete all competency assessments.
    • Stay current with pharmacy practices and seek professional development.

Qualifications:

  • Education: Graduate of an ACPE-accredited pharmacy college.
  • Licensure: Currently licensed or eligible for licensure in Kentucky and Indiana (must obtain Indiana licensure within 90 days of hire if licensed in Kentucky).
  • Experience: At least 6 months of experience as a pharmacist in a hospital or comparable setting.
  • Skills:
    • Problem-solving and critical thinking.
    • Effective communication skills.
    • Familiarity with current practice standards (ASHP).
