Job Summary:

The Solid Organ Transplant Clinical Pharmacist provides specialized, patient-centered clinical services to the inpatient solid organ transplant population as well as the outpatient transplant clinic and infusion center. This position focuses on promoting optimal therapeutic outcomes, cost-effective drug therapy, and effective teamwork while supporting the development and implementation of treatment protocols. The pharmacist will assist with the verification and dispensing of medication orders, educate healthcare professionals on pharmacotherapy, and work collaboratively on multidisciplinary drug therapy management plans. A commitment to teamwork, customer service, and continuous improvement is essential.

Key Responsibilities:

  • Clinical Pharmaceutical Care:

    • Provide pharmaceutical care to solid organ transplant patients, specifically those undergoing abdominal, heart, and lung transplants (both inpatient and clinic settings).
    • Promote quality care delivery and optimal therapeutic outcomes for transplant patients.
  • Pharmacy Services:

    • Demonstrate proficiency in pharmacy distributive services while maintaining focus on patient care.
    • Support the development and implementation of treatment protocols aimed at improving patient care and therapeutic outcomes.
  • Education & Professional Development:

    • Educate other healthcare professionals regarding pharmacotherapy management in solid organ transplant cases.
    • Contribute to the professional development of the Pharmacy department, supporting continuous learning and skill enhancement.
    • Engage in self-development activities to stay updated on the latest in transplant pharmacy care.
  • Team Leadership:

    • Oversee the daily activities of the transplant transitions of care technician.
    • Foster a cooperative and collaborative work environment, using communication and team-building skills to enhance patient care.
  • On-Call and Weekend Rotation:

    • Rotate every 6th weekend to provide clinical transplant pharmacy services.
  • Other Duties:

    • Perform other duties as assigned by the leadership team.

Compensation:

  • Hourly Rate: $52.46 - $83.94

Minimum Qualifications:

  • Education:

    • Bachelor’s in Pharmacy or Pharm. D Degree required.
  • Experience:

    • PGY-1 residency required or equivalent hospital experience (minimum of 3 years).
    • PGY-2 residency is preferred, especially for specialty areas.
  • Licensure:

    • Virginia State Board of Pharmacy License required.
    • American Heart Association (AHA) Health Care Provider BLS certification required.

Physical Demands:

  • Job requires sitting for prolonged periods and repetitive motion (typing).
  • Proficient communication, auditory, and visual skills are necessary.
  • Attention to detail and ability to write legibly.
  • Ability to lift, push, or pull items less than 20lbs.
  • May be exposed to fumes, chemicals, blood/body fluids, and infectious diseases.
