Job Title: Clinical Pharmacist

Position Details:

  • Hours: Monday-Friday, 7:00 AM - 4:00 PM during training. Must be flexible as needed.

Position Summary:
The Clinical Pharmacist is responsible for ensuring the safe, effective, and timely medication therapy for District patients. This role involves supporting medication-use systems and delivering optimal medication therapy across a variety of disease states. The pharmacist will provide direct patient care in collaboration with physicians, nurses, and other hospital staff. This position is aligned with interdisciplinary programs and specialty services to integrate pharmaceutical care into the hospital’s systems. The Clinical Pharmacist will assess patient needs, provide drug therapy education, monitor patient responses, and intervene to prevent medication-related issues. The role includes supporting quality and regulatory compliance initiatives, contributing to the Quality Management Program, and offering medication practice-related education to staff.

Requirements:

  • Education: Graduate of an ACPE accredited College of Pharmacy.
  • Licensure/Certification/Registration: Licensed (or eligible for licensure) in the state of Oregon.
  • Experience: Clinical pharmacy experience in a hospital setting preferred.
