Clinical Pharmacist II – Pharmacy
Location: San Antonio, TX

Job Summary:
The Clinical Pharmacist II is responsible for delivering compassionate, efficient, and excellent pharmacy services. This role includes traditional dispensing responsibilities, as well as active participation in pharmacotherapy management and quality medication practices within assigned service lines.


Key Responsibilities:

Operational Duties:

  • Prepare and dispense medication orders according to institutional protocols.

  • Accurately interpret and transcribe medication orders into patient records.

  • Compound and dispense sterile, non-sterile, chemotherapy, and parenteral nutrition products.

  • Manage controlled substance distribution and recordkeeping in compliance with laws and policies.

  • Supervise pharmacy technicians and oversee their daily functions.

  • Maintain current pharmacist license and continuing education records.

  • Participate in performance assessments and complete required competencies.

  • Contribute at least one medication use project annually.

Ambulatory Care Pharmacy Services:

  • Provide pharmaceutical care under protocols/collaborative agreements.

  • Manage drug therapy and anticoagulation, offer patient education, and support medication reconciliation.

  • Provide clinical services in outpatient settings (e.g., clinics, intensive medical home, transition of care).

Clinical Duties:

  • Ensure safe and cost-effective drug therapy according to protocols.

  • Monitor therapy for contraindications, interactions, allergies, and appropriate dosing.

  • Support pharmacokinetics services and renal dosing.

  • Accurately interpret patient medical records.

  • Report adverse drug reactions in a timely manner.

  • Maintain formulary compliance and support therapeutic substitutions.

  • Deliver drug consultations to clinicians and patient education as needed.

  • Participate in quality improvement, medication use evaluations, and documentation activities.

  • Engage in stewardship efforts (e.g., antibiotic, opioid) and attend interdisciplinary rounds when assigned.

Professional Contribution:

  • Precept pharmacy students and residents.

  • Assess and maintain pharmacist competencies.

  • Deliver at least one in-service per year.

  • Participate in committees and complete at least one clinical project annually.

Occupational Hazards:

  • Potential exposure to hazardous substances (e.g., cytotoxic drugs), sharps, biologics, and musculoskeletal injury if proper procedures are not followed.


Qualifications:

Education:

  • Doctor of Pharmacy (PharmD) or Bachelor of Science in Pharmacy required.

Experience:

  • PGY-1 and PGY-2 residency, or Board Certification, or a minimum of 5 years of hospital experience required.

Licenses/Certifications:

  • Active pharmacy license within 60 days of hire.

  • Pharmacist preceptor certification within 90 days.

  • BLS or ACLS within 180 days.

  • PALS required for pediatric roles within 180 days.

  • ASHP Residency Preceptor status (if applicable) within 180 days.
