Job Details:

PeaceHealth is seeking a Pharmacist for a Per Diem/Relief position (0.00 FTE) in Florence, OR. The position offers hourly compensation ranging from $64.78 to $97.18 per hour.

Key Responsibilities:

  • Medication Review: Review medication orders for appropriateness and accuracy, including contraindications, allergies, dose, route, frequency, duplicate therapy, and formulary status.
  • Order Interpretation: Clarify and rectify physician medication and IV orders as well as prescriptions, including selecting medications for outpatient dispensing.
  • Drug Therapy Monitoring: Monitor patients on specified target drugs, initiate pharmacokinetic dosing protocols, and evaluate drug therapy for appropriateness.
  • Collaboration with Clinicians: Make drug therapy recommendations to clinicians and prescribe drug therapy as per collaborative drug therapy management agreements and protocols.
  • Patient Counseling: Dispense final products for administration and counsel patients.
  • Adverse Event Reporting: Detect, monitor, and report adverse drug events and medication errors (EIR).
  • Supervision: Supervise and train pharmacy technicians to ensure accurate order entry, preparation, and technique. Supervise and train pharmacy interns and externs and may serve as a preceptor.
  • Protocol Development: Participate in the development of drug protocols and clinical pathways, and perform drug usage evaluations for selected drugs.
  • Quality Assurance: Complete quality assurance audits as required for prescriptive authority.

Required Qualifications:

  • Education: Graduate of an accredited School of Pharmacy with a BS/PharmD degree.
  • Skills and Experience:
    • Demonstrated basic pharmacy clinical skills.
    • Customer service skills.
    • Strong planning, prioritizing, and organizational abilities.
    • Excellent verbal and written communication skills.
    • Strong mathematical and analytical skills.
    • Basic computer and keyboarding skills, including MS Office applications.
    • Ability to work independently and identify medications.
  • Licensing: Currently licensed or able to obtain a Registered Pharmacist license in the state where you will be working.

Certification Requirements:

  • BLS Certification: Must be completed within 6 months of hire.
  • COVID-19 Vaccination: Required completion of the primary COVID-19 vaccine series before the start date. Medical and religious exemption processes are available.
