Job Summary

 

The Chart Scrubber at our client’s multispeciality practice will play a pivotal role in ensuring the accuracy and completeness of patient charts, with a primary focus on upcoming appointments. This individual will be responsible for reviewing and updating patient records, identifying pending tests and screenings, assessing eligibility for the Chronic Care Management Program, and coordinating routine checkups based on surgical history. The role requires meticulous attention to detail and effective communication with the healthcare team to enhance patient care.

Role and Responsibilities

Chart Review

  • Thoroughly review patient charts to identify and update previous and current medical conditions.
  • Identify pending tests, screenings, and other relevant medical information.

Chronic Care Management

  • Assess patient eligibility for enrollment in the Chronic Care Management Program.
  • Collaborate with the relevant team to facilitate patient enrollment and ongoing management.

Surgical History Check

  • Review surgical history in patient charts and inform the healthcare team about any routine checkups or follow-up appointments required.

Eligibility Checks

  • Go through patients’ insurance plans, understand the coverage of various tests and screenings and inform the relevant team to schedule appointments.

Information Accuracy

 

  • Ensure the accuracy and completeness of patient information in electronic health records.

Appointment Reminders

  • Schedule reminders for patients based on previous and upcoming tests, screenings, and routine checkups.
  • Collaborate with administrative staff to ensure timely communication of appointment details to patients.

Collaboration

  • Work closely with healthcare providers, nurses, and administrative staff to communicate relevant patient information.
  • Collaborate with the Chronic Care Management team and surgical departments as needed.

Documentation

  • Maintain detailed and organized documentation of chart reviews, eligibility assessments, and communication with the healthcare team.

Qualifications

 

Minimum 2 years of full-time work experience in a similar role

Strong written and verbal communication skills.

Bachelor/ Master’s degree

Proficient in Microsoft Office Suite and virtual collaboration tools.

Excellent time management and organizational abilities.

Demonstrated ability to identify and resolve issues proactively.

A team player who can collaborate effectively with colleagues at all levels.

Adaptability to changing priorities and tasks.

Schedule (US Shifts Only)

Eastern Time – 6:30 p.m. – 3:30 a.m. IST, Monday – Friday

Pacific Time – 9:30 p.m. – 6:30 a.m. IST, Monday – Friday
