Position Overview

As a Case Management Specialist (CMS), you will play a vital role in supporting Rare Disease patients and families through personalized care coordination and case management.
You’ll identify and overcome barriers to treatment access, ensure seamless insurance approvals, and coordinate with healthcare providers, payers, and specialty pharmacies to maintain optimal patient support.

This role is part of Sanofi’s Rare Diseases “One Team, Common Goals, Single Mission” approach — a high-performing, patient-centered model that emphasizes accountability, strategic insight, and collaboration across therapeutic areas.


Key Responsibilities

1. Patient Support & Care Coordination

  • Lead and manage the case management process with urgency and purpose.

  • Assess patient access needs and create individualized care plans for treatment initiation and continuity.

  • Facilitate insurance approvals for Sanofi Rare Disease therapies.

  • Educate patients, caregivers, and healthcare providers about insurance coverage, limitations, and support options.

  • Enroll eligible patients into financial assistance or patient support programs.

2. Healthcare System Navigation

  • Maintain deep knowledge of reimbursement processes, insurance plans, billing/coding guidelines, and payer trends.

  • Stay updated on regional healthcare resources and apply this understanding to improve patient access.

  • Ensure compliant exchange of patient information with internal and external stakeholders.

3. Cross-Functional Collaboration

  • Work closely with medical, sales, market access, and patient services teams to optimize patient outcomes.

  • Act as a key contributor to account strategy and planning, providing patient insights and market understanding.

  • Participate in launch preparations and provide insights on payer coverage, access challenges, and rare disease market conditions.

4. Relationship Management

  • Build and sustain professional relationships with patients, caregivers, providers, insurers, specialty pharmacies, and infusion site staff.

  • Represent Sanofi at patient meetings, site visits, conferences, and trade shows, promoting awareness of case management services.

5. Data Management & Reporting

  • Maintain accurate, timely documentation of all patient interactions, coverage approvals, and follow-ups in the CRM system (Salesforce preferred).

  • Use data analytics and reporting tools to make informed territory decisions and track performance indicators (KPIs).

6. Leadership & Continuous Improvement

  • Demonstrate accountability, innovation, and proactive problem-solving.

  • Share knowledge through training, case studies, and special projects within the Patient Support Services (PSS) team.

  • Identify process gaps and propose solutions to enhance program effectiveness.


Qualifications

Required

  • Bachelor’s Degree

  • 3–5 years of patient-facing or high-touch customer interaction experience

  • Strong understanding of health insurance benefits, reimbursement processes, and relevant laws/regulations

  • Excellent communication, mediation, and problem-solving skills

  • Ability to work effectively within a complex matrix organization

  • High emotional intelligence — flexibility, empathy, and persistence

  • Proficient with MS Office and data entry tools

  • Strategic thinking and strong business acumen

  • Ability to travel up to 10%, including occasional weekends

Preferred

  • Bilingual (Spanish)

  • Salesforce CRM experience

  • Prior work in case management, insurance, or healthcare systems

  • Experience in pharma/biotech patient services


Working Conditions

  • Remote role – working hours must align with the supported geography.

  • Occasional travel for meetings, conferences, or training sessions.
