Position Overview:
We are seeking a highly motivated Ambulatory Clinical Pharmacist to support the Endocrine and Diabetes clinic. This on-site role in West Reading, Pennsylvania involves direct patient care and close collaboration with physicians and clinic staff. The ideal candidate will have a robust clinical pharmacy background and knowledge of retail pharmacy operations.


Work Schedule:

  • Monday to Friday, daytime hours

  • Typically no weekend or holiday shifts


Key Responsibilities:

  • Serve as a valuable pharmacy liaison to physician practices

  • Support clinic operations by addressing medication queries from providers (e.g., formulary options, dosing, adverse reactions)

  • Coordinate and ensure prescription flow within the ambulatory pharmacy

  • Maintain and manage patient schedules for pharmacist-led services:

    • Medication counseling

    • Pillbox refills

    • Immunizations

    • Medication administration

  • Track and follow up on patient outcomes, assessing medication efficacy and safety

  • Manage prescription refills with verifiable proof-of-delivery documentation

  • Submit and manage prior authorizations, appeals, and coverage denials

  • Handle limited-distribution drugs and prescriptions outside of pharmacy network

  • Act as a financial advocate for patients by leveraging assistance programs

  • Utilize EMR and specialty therapy platforms for appropriate documentation

  • Support expansion of pharmacy services through committee involvement

  • Assist with ambulatory pharmacy staffing as needed


Education & Licensure Requirements:

  • Doctor of Pharmacy (PharmD) degree required

  • PGY1 residency or ambulatory/diabetes management experience preferred

  • Unrestricted PA pharmacist license, including immunization authority

    • Out-of-state candidates must obtain PA licensure within 90 days of offer

  • 340B program experience is a plus


Skills & Experience:

  • Strong clinical expertise in chronic disease management

  • Knowledge of prescription processing and claims adjudication in retail settings

  • Proficient with Epic or similar EMR systems

  • Comfortable interpreting clinical data, lab results, and writing patient notes

  • Microsoft Office proficiency (Word, Excel, PowerPoint)
