About the Role:
As a Population Health Coordinator at Optum, you will play a crucial role in improving patient outcomes by supporting clinical teams with outreach, coordination, and data-driven processes. You’ll work closely with clinicians, care teams, and patients to ensure proactive care delivery, particularly for those overdue for lab work or medical appointments.
Work Schedule:
Full-time: 38.75-hour workweek
Monday – Friday | Business hours: 8:00 AM – 4:30 PM EST
Hybrid model: 3 days in-office, 2 days remote (subject to team needs)
Occasional overtime may be required based on business demand
Primary Responsibilities:
Conduct patient outreach via phone, email, and/or mail to promote clinical follow-ups, lab testing, and appointment attendance.
Place lab orders and schedule appointments in the Epic EMR system as directed by clinicians.
Generate and analyze reports in Healthy Planet Registry to identify patients needing labs or overdue screenings.
Collaborate with clinicians to determine population reports and plan outreach strategy.
Coordinate with care teams to support patient care plans and assist in service linkages.
Identify and report data gaps, trends, and improvement opportunities.
Perform other administrative and clinical support duties as assigned.
Required Qualifications:
High school diploma or GED (or higher).
Availability to work a full-time schedule (Monday – Friday, 8:00 AM to 4:30 PM EST).
Reside within commuting distance to Worcester, MA.
Quiet, secure home workspace with a high-speed internet connection.
Ability to maintain Protected Health Information (PHI) confidentiality.
Preferred Qualifications:
1+ year of experience in healthcare or medical coordination roles.
Familiarity with electronic medical records (EMR) and medical terminology.
Proficiency in Microsoft Office and spreadsheet data manipulation.
Experience in working with data reports or registries.
Ability to travel locally up to 50% of the time (if required by the team).
