About the Role:
As a Population Health Coordinator at Optum, you will play a crucial role in improving patient outcomes by supporting clinical teams with outreach, coordination, and data-driven processes. You’ll work closely with clinicians, care teams, and patients to ensure proactive care delivery, particularly for those overdue for lab work or medical appointments.

Work Schedule:

  • Full-time: 38.75-hour workweek

  • Monday – Friday | Business hours: 8:00 AM – 4:30 PM EST

  • Hybrid model: 3 days in-office, 2 days remote (subject to team needs)

  • Occasional overtime may be required based on business demand

Primary Responsibilities:

  • Conduct patient outreach via phone, email, and/or mail to promote clinical follow-ups, lab testing, and appointment attendance.

  • Place lab orders and schedule appointments in the Epic EMR system as directed by clinicians.

  • Generate and analyze reports in Healthy Planet Registry to identify patients needing labs or overdue screenings.

  • Collaborate with clinicians to determine population reports and plan outreach strategy.

  • Coordinate with care teams to support patient care plans and assist in service linkages.

  • Identify and report data gaps, trends, and improvement opportunities.

  • Perform other administrative and clinical support duties as assigned.

Required Qualifications:

  • High school diploma or GED (or higher).

  • Availability to work a full-time schedule (Monday – Friday, 8:00 AM to 4:30 PM EST).

  • Reside within commuting distance to Worcester, MA.

  • Quiet, secure home workspace with a high-speed internet connection.

  • Ability to maintain Protected Health Information (PHI) confidentiality.

Preferred Qualifications:

  • 1+ year of experience in healthcare or medical coordination roles.

  • Familiarity with electronic medical records (EMR) and medical terminology.

  • Proficiency in Microsoft Office and spreadsheet data manipulation.

  • Experience in working with data reports or registries.

  • Ability to travel locally up to 50% of the time (if required by the team).
