Job Summary
Optum is seeking a Manager – Social Work to lead a team of on-site hospital social workers within the Care Coordination Department in Waukesha, WI. This leadership role involves managing case management workflows, ensuring compliance with Medicare and Medicaid regulations, mentoring staff, and implementing care transition strategies to improve patient outcomes. The ideal candidate will have a strong background in hospital social work, case management, and team leadership.
Key Responsibilities
Supervise and coordinate daily workflow activities of hospital social workers.
Implement departmental strategies to address operational challenges and enhance care coordination.
Provide social work expertise and support to UM/CM activities.
Mentor and train social work team members for professional growth.
Build and maintain strong relationships with internal teams and external healthcare partners.
Develop and enforce policies, procedures, and compliance protocols.
Facilitate regular team meetings to discuss complex cases and operational updates.
Oversee HR activities including staff performance, development, and evaluations.
Prepare and submit operational and performance reports to senior management.
Required Skills & Qualifications
Licensed Social Worker in WI (BSW, MSW, LSW, or LCSW).
2+ years of experience in case management.
Experience in electronic documentation systems.
Strong understanding of Medicare and Medicaid compliance.
Proficiency in Microsoft Word, Excel, and PowerPoint.
Ability to work on-site in a hospital environment daily.
Preferred Qualifications
Case management certification.
Experience with EPIC EMR systems.
Previous team management or leadership experience.
Hospital or acute care social work experience.
