Bilingual English/Spanish Community Health Worker (Service Coordinator L3)

Location: El Paso, TX (On-site – 1145 Westmoreland Dr, El Paso, TX 79925)

Compensation: $20.00 – $35.72 per hour (based on experience & eligibility)
Schedule: Full-time, Monday–Friday, 8:00 am – 5:00 pm MST (occasional overtime)


Job Summary

Optum is hiring a Bilingual (English/Spanish) Community Health Worker / Service Coordinator Level 3 to deliver person-centered care coordination for STAR+PLUS members in El Paso, TX. You’ll assess, plan, implement, and monitor Long-Term Services & Supports (LTSS) to keep medically complex, ABD/SSI members healthy, independent, and safely living in the community. If you’re passionate about care management, Medicaid/Medicare populations, social determinants of health (SDOH) and advocacy, this on-site role is for you.


Key Responsibilities

  • Perform telephonic assessments and ongoing care coordination for Level 3 STAR+PLUS members

  • Develop individualized, person-centered plans of care in collaboration with members, families & providers

  • Identify and coordinate LTSS, community, psychosocial, financial and state support services

  • Serve as a single point of contact across the continuum of care; monitor timeliness & appropriateness of services

  • Maintain electronic charting and documentation per regulatory requirements (HIPAA)

  • Provide administrative support to Level 1 & 2 Service Coordinators (RN & SW) as needed

  • Ensure strict adherence to STAR+PLUS contract requirements and quality standards

  • Refer members to disease management programs and escalate medically complex cases


Required Skills & Qualifications

  • High School Diploma/GED (or equivalent experience)

  • Bilingual fluency in English & Spanish (must-have)

  • 4+ years in community health within a healthcare role

  • Direct ABD/SSI population experience in 3 of the last 5 years

  • Experience in long-term care, home health, hospice, public health or assisted living

  • 1+ year working with Microsoft Word, Excel, Outlook & electronic charting

  • Strong knowledge of case management processes, person-centered care, Medicaid/Medicare rules

Preferred

  • Service coordination or case management experience (e.g., LTSS coordinator with DADS/DSHS)

  • Experience with Texas STAR+PLUS, managed care, or dual-eligible (Medicare/Medicaid) populations

  • Prior roles in utilization review or health plan operations
