Job Title: Patient Care Coordinator

Company: Large Managed Care Organization
Location: 15320 Amberly Dr. Suite B, Tampa, FL 33610
Pay: $19.00 per hour
Contract Dates: 8/11/24 - 11/11/24
Work Hours: Monday to Friday, 8 AM - 5 PM (40 hours/week), with 1-hour lunch. Flexible hours available between 8 AM and 6 PM.

Position Overview:
We are seeking Patient Care Coordinators to manage the initial triage of members, facilitate administrative intake, and oversee the admission and discharge processes.

Key Responsibilities:

  • Determine member eligibility for services.
  • Process referrals for patients.
  • Provide and explain benefit information to members and providers.
  • Obtain authorizations from the Health Plan.
  • Schedule appointments for members based on their requests.
  • Address calls and inquiries from members and providers regarding case status.
  • Utilize the phone system to respond to and transfer calls to the appropriate personnel.
  • Coordinate with specialists and patients to arrange appointments.
  • Serve as the primary point of contact for providers and members regarding medical, behavioral, or clinical services and benefits.
  • Extract and review fax requests for medical or clinical services.
  • Handle calls requesting information about medical, behavioral, or clinical services and benefits from providers and members.
  • Receive electronic referral form requests for medical or clinical services.

Qualifications:

  • High School Diploma or GED equivalent.
  • Experience with healthcare EMR systems is required.
  • Ability to navigate between computer screens and platforms to research information (e.g., medical, clinical, or benefits information).
  • Some understanding of the patient referral process is helpful, but not required; we are willing to provide training.
