About the Role
We are seeking a detail-oriented and experienced Medical Records Coding & Transcription Senior Representative to support healthcare documentation accuracy, medical coding operations, and clinical records standardization within a healthcare operations environment. This role is ideal for professionals with expertise in medical coding, medical records review, clinical documentation interpretation, healthcare transcription, and coding compliance.

The ideal candidate will review clinical documentation, interpret patient diagnoses and procedures, assign standardized medical codes, and support accurate healthcare documentation processes to ensure consistency, compliance, and operational efficiency.

Key Responsibilities

Medical Coding & Clinical Documentation Review

  • Review patient medical records, physician documentation, discharge summaries, clinical notes, and healthcare records to identify accurate diagnoses, procedures, treatments, and services.
  • Interpret clinical documentation to assign appropriate standardized medical codes.
  • Ensure coding accuracy, specificity, completeness, and compliance with healthcare documentation standards.
  • Support consistent and high-quality clinical documentation practices across healthcare workflows.

Diagnosis, Procedure & Service Coding

  • Convert healthcare diagnoses, procedures, medical services, treatments, and equipment documentation into universal alphanumeric medical codes.
  • Apply coding standards such as ICD, CPT, HCPCS, or relevant healthcare coding frameworks depending on organizational requirements.
  • Ensure standardized coding for billing, reporting, reimbursement, analytics, and healthcare records management.

Medical Transcription & Documentation Support

  • Support medical transcription-related documentation activities where applicable.
  • Review transcribed clinical content for completeness, terminology accuracy, and coding readiness.
  • Ensure consistency between clinical documentation and coded healthcare records.

Healthcare Data Quality & Compliance

  • Maintain high standards of documentation accuracy, data integrity, and coding quality.
  • Ensure coding practices comply with organizational guidelines, healthcare regulations, coding standards, and documentation policies.
  • Support audit readiness and quality monitoring initiatives.
  • Identify discrepancies, incomplete records, or documentation gaps requiring clarification.

Clinical Terminology Interpretation

  • Apply strong knowledge of medical terminology, anatomy, physiology, disease processes, injuries, procedures, and healthcare documentation standards to interpret records accurately.
  • Maintain understanding of evolving coding standards and healthcare documentation practices.

Process Improvement & Operational Excellence

  • Contribute to workflow efficiency, coding productivity improvements, and documentation quality initiatives.
  • Escalate operational challenges, coding concerns, or documentation quality issues proactively.
  • Support team adherence to productivity, quality, and compliance expectations.

Required Qualifications

  • Bachelor’s degree or diploma in Life Sciences, Pharmacy, Nursing, Healthcare, Medical Coding, Health Information Management, Biotechnology, or related disciplines preferred.
  • 2–5+ years of relevant experience in medical coding, healthcare documentation, medical records review, or medical transcription environments.
  • Strong understanding of medical coding systems, healthcare terminology, anatomy, physiology, clinical documentation, and coding compliance standards.
  • Experience interpreting physician notes, clinical documentation, diagnoses, and procedural records.
  • Familiarity with ICD, CPT, HCPCS, EMR systems, and healthcare documentation platforms is preferred.
  • Excellent attention to detail, analytical thinking, accuracy, and documentation review skills.
  • Strong written and verbal communication abilities.
