Job Summary:

We are seeking a skilled and detail-oriented Medical Coder – E&M to accurately code medical records for Evaluation and Management services. The ideal candidate will ensure compliance with regulatory requirements, support billing accuracy, and collaborate effectively with clinical and administrative teams.


Key Responsibilities:

Coding & Documentation:

  • Analyze medical records to identify services provided during patient evaluations and management.

  • Assign accurate CPT, ICD-10-CM, and HCPCS codes based on documentation and coding guidelines.

  • Ensure documentation meets requirements for various E&M levels.

  • Conduct regular audits and quality checks to identify errors or inconsistencies.

Compliance & Regulations:

  • Adhere to HIPAA and other regulatory standards to ensure confidentiality and data security.

  • Stay current with coding regulations and updates from CMS, AMA, and other governing bodies.

  • Participate in internal coding compliance programs to maintain quality standards.

Collaboration & Communication:

  • Work closely with physicians, nurses, and healthcare staff to clarify documentation and obtain necessary information.

  • Coordinate with billing and revenue cycle teams to ensure clean claim submissions.

  • Respond to coding-related queries and resolve discrepancies with providers.

System & Process Management:

  • Utilize coding software and Electronic Health Record (EHR) systems proficiently.

  • Follow best practices and maintain thorough knowledge of coding conventions and principles.


Qualifications:

Certifications:

  • Certified Professional Coder (CPC) or equivalent credential (e.g., CCS-P, CRC).

Experience & Skills:

  • Strong knowledge of E&M coding guidelines.

  • Proficient with medical coding tools and EHR systems.

  • Familiarity with medical terminology, anatomy, and physiology.

  • High level of attention to detail and strong analytical skills.

  • Excellent communication and interpersonal abilities.

  • Self-motivated and able to work both independently and in a team.

  • Strong time management and organizational capabilities.
