Are you a skilled and dedicated Clinical Pharmacist seeking to make a direct impact in a hospital setting? Join our multidisciplinary team at McCullough Hyde Memorial Hospital and play a pivotal role in ensuring safe, effective, and individualized medication therapy for our patients.

As a Clinical Pharmacist, you will collaborate with physicians, nurses, and other healthcare professionals to optimize medication therapy and enhance patient outcomes. You’ll provide critical expertise in drug therapy management, medication reconciliation, and patient education, ensuring our patients receive the highest standard of care.

Key Responsibilities:

  • Collaborate with Healthcare Teams: Work closely with physicians, nurses, and other healthcare providers to ensure appropriateness of medication therapy.
  • Patient Care: Conduct careful reviews of patient medication, assess for potential issues, and provide pharmaceutical recommendations to improve patient care.
  • Medication Safety: Ensure the 7 patient "rights" (right patient, dose, drug, route, time, information, and documentation) are respected, report medication errors, and assist in risk management strategies.
  • Inventory Management: Manage drug inventory, adjust par levels, and communicate with Pharmacy Buyers to meet demand while minimizing waste.
  • Precepting & Education: Assist with orientation, training of new hires, and precepting pharmacy students and residents.
  • Pharmacy Process Improvement: Identify and address opportunities for pharmacy process improvements and contribute to clinical pharmacy program development.

Minimum Requirements:

  • Bachelor’s Degree in Pharmacy
  • Active pharmacist license in the state of Ohio
  • 1-2 years of experience as a pharmacist (inpatient or infusion pharmacy experience preferred)
  • Strong working knowledge of Ohio Board of Pharmacy regulations
  • Prescription formula and mathematics skills for calculating dosages

Working Hours:

  • Every Tuesday and Wednesday, 7:30 AM - 6:00 PM
  • Required to work every 3rd weekend and holiday
