Role Overview

The Pharmacist will be working in the Sutter Sync program, a comprehensive care management initiative designed to remotely manage patients with chronic conditions such as hypertension, diabetes, and other chronic diseases. Pharmacists will collaborate with Health Coaches to manage a panel of patients, providing medication adjustments and personalized lifestyle advice. The position combines remote patient care with an onsite presence during the program’s startup phase at the Innovation Center in San Francisco, with the potential for remote work afterward.

The role also includes shaping workflows, creating educational materials, and assisting in onboarding new pharmacists as part of the program launch in March 2025.


Key Responsibilities

  • Pharmaceutical Care: Deliver patient-centered clinical services, including medication management, education, preparation, and distribution to ensure safe, effective, and cost-efficient medication therapy.
  • Collaboration: Work alongside Health Coaches and healthcare providers to adjust medications, provide lifestyle advice, and implement the patient’s care plan based on diagnosis.
  • Patient Engagement: Build rapport with patients and their families through attentive monitoring, care, and effective communication.
  • Regulatory Compliance: Ensure adherence to local, state, and federal regulations regarding privacy and patient safety.
  • Startup Support: Assist with the creation and refinement of pharmacist workflows, and develop educational materials for the program, particularly as new pharmacists are onboarded.
  • Problem Solving: Use professional judgment and clinical expertise to resolve complex problems in daily operations.

Education & Certifications

  • Education: Doctor of Pharmacy (PharmD) degree from an accredited college of pharmacy.
  • Certification & Licensure: Active PharmD-Licensed Pharmacist status.

Experience Requirements

  • Experience: At least 3 years of recent, relevant experience in a pharmacy role.

Compensation

  • Hourly Pay Range: $100.70 to $120.84, depending on experience and qualifications.

Work Location

  • Onsite: Initially at the Sutter Health Innovation Center in San Francisco, with potential remote work once the program stabilizes. Additional onsite presence may be required as new conditions are added.
